All information on the nomination form must be completed accurately and in full for the nominee to be considered.
Only one category may be selected per nomination. Please choose the most relevant category for your nominee.
Nominators may submit more than one nomination form. Feel free to nominate as many deserving HERoes as you'd like!
Self-nominations are allowed; individuals are encouraged to nominate themselves if they believe they are deserving.
Nominators are required to provide a maximum of 250 words explaining why the nominee deserves the recognition. Be sure to capture the nominee's impact, accomplishments, and contributions.
There is no maximum limit to the number of nominations a nominee can receive. A nominee can be recognized for multiple categories based on their diverse contributions.
Winners will be selected by members of the HER Collective.
Winners will be notified on or before October 31, 2023, via the contact information provided on the nomination form.
